Please bring the completed forms with you to your scheduled appointment with Dr. Stram, Dr. Netter, Dr. DiRoma or Jennifer Adams, NP only. These forms are not necessary if you are meeting with one of our other practitioners.

Integrative Medicine is a patient centered approach to healthcare that attends to the whole person- body, mind, heart and spirit. We consider the relationship that develops between client and practitioner an essential part of the therapeutic experience. This requires a commitment of time and effort from both parties. To begin this process we ask that you complete the comprehensive intake form and bring it with you to your first visit with Dr. Stram, Dr. Netter, Dr. DiRoma or Jennifer Adams, NP.

You may feel some of the questions are unusual, too intimate or even unnecessary for a medical intake form. However, these questions are at the heart of why Integrative Medicine works, as we begin to get to the heart of whom you are and what you need to obtain optimal wellness.

In addition to our medical intake form, we would like to remind you to bring or fax any pertinent medical records or recent tests to this appointment.
